Articular dislocation of hip
Hip dislocation can be a simple dislocation or it can be a fracture dislocation that involves the femoral head or the post wall of the acetabulum.
There are 2 types of hip dislocation
- Posterior hip dislocation -dislocation of the hip is usually a posterior type however in rare situations it can be an anterior dislocation.
• Posterior hip dislocation is the most common
• Lower limb will be flexed, adducted, and internally rotated. - anterior hip dislocation – it’s rare
• lower limb will be extended abducted and externally rotated.
• Anterior inferior dislocation (obturator type)

Symptoms:-
The patient can’t move the leg
• Loss of sensation in the foot and ankle
• Pain
• Muscles shortening and weakness
• Swelling
Hip dislocation of any type must be reduced in less than 6 hours of injury.
• After reduction of the hip get a CT scan to see any loose fragments in the joints
• You will get an X-ray first then get a CT scan.
• CT scan will clearly outline the bony injury.is helpful to check for congruous reduction for the absence of fracture, absence of marginal impaction, absence of loose fragments pieces inside the joint
• Hip dislocation with or without associated fracture can cause complications
• AVN (avascular necrosis)
• The risk of avascular necrosis depends on the interval between the injury and reduction of the dislocation.
• Urgent reduction of hip dislocation is mandatory to void these complications.
• Post hip dislocation causing sciatic nerve palsy – reduce the hip and recheck the sciatic nerve function always reduce the hip early.
• Closed reduction should be done in less than 6 hours to check the sciatic nerve before reduction and after reduction.
• Peroneal nerve affected – when an injury occurs to the sciatic nerve due to posterior hip dislocation.
• The common peroneal nerve in usually affected causing weakness in dosri flexion of the ankle and foot of or extension.
• Pipkin fracture – hip joint dislocation may be associated with an acetabular fracture or the fracture of the femoral head.
Treatment of hip dislocation
• Do emergency closed reduction of the hip with 6 hour
• Early closed reduction is done to avoid avascular necrosis of the hip.
• After reduction of the hip joint,do mobilization of the patient with protected weight-bearing crutches for approx 4 weeks.
