The grading system of knee osteoarthritis
Grade 0:- In this stage the knee is healthy.
Grade 1 – In this stage Very minor bone spur develops but the patient is not experiencing any pain or discomfort.
Grade 2 – During this stage people start experiencing symptoms of OA. It is a mild stage and bone spur can be observed through x-rays but the cartilage remains at a healthy size.
Grade 3 – During this stage OA is in moderate condition. Cartilage damage can be observed along with slightly reduced joint space through x-ray.
Grade 4 – This is the most severe stage of OA. The joint space between the knee joint is dramatically reduced, the cartilage will almost be completely gone and the synovial fluid will be decreased. This stage is normally associated with high levels of pain and discomfort during joint motion.

Observation of the knee – The knee may be enlarged, swollen, or red if the OA is very reactive or irritated.
Gait assessment – To observe any stiffness during gait or any significantly reduced weight bearing of the affected knee.
