First, we have to know what is Traction
Traction
Traction ( Decompression therapy ) is the process of applying manual or mechanical force to the body in such a way that it separates the joint surface, elongates soft tissue, or aligns the bone fragment.
Type of traction
On the nature of the pull
(a) Continuous traction –
This method of traction is mainly used for temporary immobilization of a symptomatic spinal area thereby relieving symptoms that get aggravated by spinal movement.
In this, a low force is applied for a longer duration ranging from 1 hour to 1 day, and is commonly practiced for acute neck and back pain. At present this method is rarely used because most patients with spinal pain do not benefit from prolonged bed rest or inactivity.
(b) Sustained traction or Static traction
– In this moderate force is applied and the pull is maintained for 20-60 minutes. The frequency of treatment is variable.
The common practice is to treat inpatients on a daily basis and outpatients 3 times a week.
(c) Intermittent traction –
This traction technique allows the application of greater force for a short period of time.
The force is gradually increased and decreased during each treatment cycle and can be administered by a pulley or motorized system.
2. On the method of application
(a) Manual –
Manual traction is applied through the hand of a therapist or a belt to pull.
It is usually applied for a few seconds or can be applied as a sudden, quick thrust.
(b) Positional–
This form of traction is applied by placing the patient in various positions using pillows, blocks, etc to provide a longitudinal pull on the spinal structures.
It usually incorporates lateral bending and is only applied to one side of the spinal segment.
(C) Inversion –
This involves using a chest harness to secure the patient as the treatment table is tilted to a vertical position, thereby using the weight of the lower half of the body to provide a traction force.
(d) Mechanical –
This involves the use of a specialized device to provide a pull.

The patient lies on a frictionless surface in a supine position and a traction force is applied in such a way that it creates a space between the joint and stretches the soft tissue resulting in the Decompression of the painful joint caused due to the nerve being pinched, reducing spasm by stretching the soft tissue or by breaking the scar tissue and align the structure.
