Type of Tendonitis
- Non-insertional Achilles tendonitis – This type of Tendonitis occurs due to small tears in the middle fibers of the tendon resulting in pain and swelling. This type of tendonitis usually affects active, younger people.
- Insertional Achilles tendonitis – This type of Tendonitis occurs in the spot where the Achilles tendon attaches to the heel bone due to the formation of bone spurs.

Diagnosis of Achilles Tendonitis
- Observation – The therapist observes muscle atrophy, swelling, joint erythema, and anatomical deformities such as heel varus, forefoot, and foot pronation which are often associated with Achilles Tendonitis along with ROM, strength, and flexibility which is limited.
- Palpation – The therapist palpates along the path of the Achilles tendon to locate the pain spot, and nodules and measures the thickness.
- Special test
- Arc test – In this test the patient lies in a prone position with the feet hanging on the edge and is asked to perform plantarflexion and dorsiflexion if the tender spot moves along with the movement of the ankle then the test is considered positive.
- Royal London hospital test – The patient lies in a prone Position with the feet hanging on the edge and is asked to perform maximum dorsiflexion and Plantarflexion and the therapist palpates the tender spot the test is considered positive if the pain decreases.
Management of Achilles Tendonitis
- RICE protocol – provided within the first 24-48 hours of injury.
- Rest – To prevent worsening the injury the affected Part should be completely rested.

- Compression- To control swelling an elastic bandage is used to provide a comfortable compression force without causing pain or constricting blood vessels to the point of occlusion
- Elevation- To reduce swelling elevate the affected ankle above the level of the heart by doing this the venous return to the systemic circulation is increasing, and the hydrostatic pressure is reduced thereby reducing edema and facilitating waste removal from the site of injury.
- Exercise therapy – gentle stretch and strengthening (eccentric) exercises can help reduce the load on the Achilles tendon such as Foam roller tender massage, Standing soleus and gastrocnemius stretch, Heel raise, Eccentric heel raise, etc.
- Electrotherapy – IFT, and US can be used to relieve pain and reduce inflammation.
