Carpal tunnel syndrome is a nerve impingement disorder that results from the compression of the median nerve which passes through a narrow passage called the carpal tunnel.

This compression typically causes pain, numbness, and tingling in the thumb, index, middle, and half of the ring finger.

Risk factor                                       

  • Gender – women are more likely to develop CTS compares to men with a 10:1

Diagnosis of carpal tunnel syndrome

  1. Description of patient symptoms
  2. Physical exam
    1. Tinel test – In this test the therapist taps on the median nerve path if the patient experiences any symptoms the test is considered positive.
    1. Phalen’s test – In this test the patient holds his arm in front and flexes the wrist for 60 seconds if the patient experiences any symptoms the test is considered positive.

Management of CTS

  1. Rest – the affected hand should be completely rested to prevent the condition from worsening and to promote recovery.
  2. Ice – ice pack can be applied on the affected hand to cope up with pain, edema, etc.
  3. Wrist splint – can be used to stabilize the hand and prevent the hand from movement.
  4. Mobilization – help the median nerve move more freely within the confines of the carpal tunnel.
  5. Electrotherapy – TENS and EMS can be used to relieve pain and stimulate the muscle.
    1. Exercise – stretching and strengthening exercises can be performed under the observation of a therapist to relieve symptoms.

    Examples – wrist flexion and extension, wrist deviation, flexion stretch, extensor stretch, etc.
