CTEV/ CONGINETAL TALIPES EQUINO VARUS/ CLUB FOOT
A deformity in the foot causes elevation at the heel making the foot turning inwards, which is present since birth.

Components –

• Hind foot equinus
• Hind foot varus
• Mid/forefoot adductus
• Cavus

Features –

1. Equinus-
• Severe tibio-talar & talo-calcaneal plantar flexion

2. Adductus-
• Medial inclination of talar neck
• Medial displacement of navicular and cuboid
• Adducted calcaneus
• Adducted forefoot as compared to the hindfoot

3. Varus–
• Adducted, plantar flexed, and inverted calcaneus

4. Cavus-
• The First metatarsal is plantar flexed

Clinical features-

• The medial and posterior ligaments of the ankle joint are very thick and taut making the foot equinus, navicular, and calcaneus adducted and inverted.
• The leg muscles work in coordination with the deformity leading to an inversely proportional relationship between the muscles and the deformity.
• Tibialis posterior is shortened because of the lengthening of gastrosoleus and toe flexors.
• The tendo Achilles and deep fascia are also affected

Effects-

• Can make the affected child permanently disabled
• May lead to physical dependence
• Underdeveloped calf muscles
• Shortening of leg or foot muscles
• Short leg length and calf length
• Difficulty in walking
• Different shoe sizes for both the feet
• Changes in developing arthritis are usually high
• Abnormal gait and balance issues

Treatment-

Operative treatment –
• It involves surgical release and repositioning
• Osteotomies
• Triple arthrodesis
• TA lengthening
• Ilizarov fixation
• JESS fixation

Non-operative treatment-
Although the incidence is rare but can make you completely dependent if not treated properly.
If left untreated can lead to adaptive changes in the bones.

After cast removal, the infant is advised certain types of shoes and brace.
