TURF TOE

Partial tear of the Turf toe is an injury to the joint of the big toe of your foot known as the First metatarsophalangeal (MTP) joint. This injury is the hyperextension injury due to which the plantar capusuloligamentous complex of your foot gets a partial tear or complete disruption. This injury has been caused by the overload on the hallux MTP joint in hyper-dorsiflexion position, this usually occurs when one player falls on another player’s heel.

Clinical characteristics:

  • Pain as a first symptom
  • Localized swelling
  • Ecchymosis
  • Stiffness of the joint
  • Grades of this injury:
    1. Grade I:
      • Local swelling
      • Plantar structures attenuation or stretching
      • Minimal ecchymosis.

     

    1. Grade II:
    • plantar structures
    • Restricted motion as a result of pain.

     

    1. Grade III:
    • Indicative swelling and ecchymosis
    • Total disruption of the plantar structures
    • Weakness of the hallux flexion
    • High instability of the MTP joint

Risk factors include:


  competing or training on artificial turf

  pes planus or excessive pronation

  decreased pre-injury ankle range of motion

  decreased pre-injury MTP joint range of motion

  soft flexible footwear.

Treatment

For low-grade Injuries :

  • perform gentle ROM.
  • Motion is limited during athletic activity by taping or
  • shoe modification.

For moderate and severe Injuries:

  • A brief period of casting with a solid walking cast,
  • A removable walking boot
  • ROM exercises
  • protected weight-bearing.

Prevention of Turf Toe

  • use of supportive footwear (with avoidance of overly flexible shoe forefoot)
  • firm inserts

avoidance of hard playing
