See an orthopaedic surgeon for fractures, suspected ligament or tendon ruptures, joint problems that have not responded to conservative treatment, and to discuss joint replacement. Most back, neck and joint pain does not need surgery, and physiotherapy is usually the first line rather than the last resort.
Orthopaedics is where most of our patients come from, and where most of them go back to. An orthopaedic surgeon deals with bones, joints, ligaments and tendons: diagnosing them, operating on them where operating helps, and deciding when it does not.
That last part matters more than people expect. A good orthopaedic surgeon turns down a great deal of surgery.
When to see one
- A suspected fracture, or an injury from a significant fall
- A joint that locks, gives way, or cannot bear weight
- A suspected ligament rupture — an ACL, a complete rotator cuff tear
- Pain that has not improved after a proper course of physiotherapy
- To discuss a knee or hip replacement
- Any deformity that is progressing
How physiotherapy fits
Physiotherapy sits on both sides of orthopaedic care.
Before. Most musculoskeletal problems should be treated conservatively first, and a large share of them resolve there. Going into surgery stronger — "prehabilitation" — also measurably improves how a joint replacement recovers.
After. The operation fixes the structure. How far the joint bends, how well it straightens and how confidently you walk are decided in the weeks afterwards. Our units are inside the hospitals where these operations happen, which is why our therapists commonly see patients from the second day rather than the sixth week.
